Please bring the completed form to your pet's first visit or fax it to us at 843-661-7398. You may also wait and fill out this form at our office, but we ask that you please arrive early so all daily appointments can begin on schedule.

Your pet can't talk, so it's up to us to figure out how they are feeling! To do so, we will perform a complete wellness exam the first time we meet your pet. We can find out so much simply by looking at their eyes, ears, nose and mouth, listening to their heart and lungs, and feeling their abdomen, joints and muscles.

We may discuss additional services such as vaccines, lab work and parasite testing based on your pet's medical history. For this reason, please bring all medical records you have with you!
